Currently, the best source of Korean and English application forms and guidance for submission to TRC 3 is the Consulate General of the Republic of Korea in Atlanta.

Some Korean Consulates / Embassies are referring Korean Adoptees to the TRC 3 website for forms, but currently the best source of KOREAN and ENGLISH application forms (BOTH of which you will need) is the Consulate General of the Republic of Korea in Atlanta.

*So far, Atlanta’s is the only Korean consulate / embassy website which has English and Korean forms in both WORD and PDF format. The WORD document is nice because it is easy to fill out by typing — however, please note that if you are submitting your final application packet via email, you will need to create a SINGLE PDF with ALL of your relevant documents. Also be sure to HAND SIGN and also date the English form. While you do NOT need to fill out the Korean form, you WILL need to sign it. Presumably if you submit your documents in person to a Korean Consulate or Embassy, you can submit a printed packet of information.
