There are blank WORD and PDF documents in ENGLISH and KOREAN which you can download from the link above. These are better formatted than the application forms from the TRC 3 website!
*PLEASE NOTE:
Korean Adoptees must FULLY complete (fill in) the ENGLISH application for TRC 3.
However, Korean Adoptees MUSTALSO HAND SIGN and DATE the KOREAN form, according to the guidance provided by the Consulate General of the Republic of Korea in Atlanta.

*PLEASE NOTE:
If you complete the form in WORD format, you MUST convert your final document to PDF format for submission to TRC 3 if submitting via email!
IF submitting by email, your final submission packet for TRC 3 should be ONE PDF containing your application and other IMPORTANT documents!
We assume that IF you submit your case through a Korean Consulate / Embassy in your home country, that you should PRINT your documents for final submission. If you are not sure, please check with your local Korean Consulate / Embassy.

PLEASE NOTE:
This page is geared toward English speaking Korean Adoptees who are submitting a NEW INDEPENDENT case to TRC 3.
If your case was one of the 311 cases suspended from TRC 2, you DO NOT need to resubmit your case.
The application for TRC 3 is just ONE part of a larger process which we have outlined here:

Do NOT simply submit your completed application form to a Korean Consulate or through email to TRC 3 UNTIL you understand the ENTIRE PROCESS, which we have outlined on the page linked above! If you do NOT understand the entire process, your application to TRC 3 will almost certainly FAIL — and you could harm the chance of others’ cases being investigated!

Please TAKE YOUR TIME to complete the application and prepare your materials for your TRC 3 submission. TRC 3 has a 2 year submission period, followed by a 1-2 year investigation period.

Please note that there is MUCH MORE to your TRC 3 application process than just completing the full English and partial Korean application forms! Please be sure to refer to the information below:

*Please note that the original PDF packet provided by the Korean Consulate in Atlanta (linked above) contains SOME pages which TRC 3 applicants DO NOT have to fill out right now.
Please NOTE CAREFULLY which forms you MUST fill out, and which forms you MUST NOT.
We recommend including ALL pages in your ENGLISH AND KOREAN applications, even if you leave the pages which you DO NOT have to fill out right now BLANK.
These pages may be needed for later TRC 3 purposes, so just to be safe, please DO BE SURE TO INCLUDE THEM in your application!

*Please note that you should HAND SIGN where indicated, instead of typing your name. You can also insert a digital signature. Please DO NOT TYPE your name as your “signature”! Please be sure to use your FULL LEGAL ENGLISH NAME throughout both the English and Korean application forms (except where otherwise noted).
We STRONGLY recommend TYPING for most of the form (except your signature — you can either hand sign or insert a digital signature). Do NOT type your name in place of a signature!
You can however NEATLY handwrite your form if necessary.
You MUST use your FULL ENGLISH LEGAL NAME throughout the TRC 3 process!
You cannot use nicknames, made up names, Korean names, etc. Please note that as part of the TRC 3 submission process, you will also need to provide documentation of LEGAL NAME CHANGES due to events such as marriage or divorce.
You must use a PERSONAL email address and PERSONAL phone number on the TRC 3 application form. You must REMAIN CONTACTABLE from 2026-2031 at this SAME PERSONAL EMAIL AND PERSONAL PHONE NUMBER!
PLEASE DO NOT APPLY TO TRC 3 IF YOU CANNOT REMAIN COMMITTED TO BEING CONTACTED AT THE SAME PERSONAL EMAIL AND PERSONAL PHONE NUMBER FROM 2026-2031!

Something IMPORTANT To Understand About The TRC 3 Process.
Once you have completed and hand signed the application form where required, be sure to SCAN — DO NOT PHOTOGRAPH — your final TRC 3 application form.
Remember that you will also need to complete certain parts of the KOREAN form. If you are not certain about how to do this, along with your COMPLETED ENGLISH application, documents, and photo ID, take a BLANK printed copy of the KOREAN form to your nearest Korean Consulate / Embassy when you are ready to apply to TRC 3 and ask them where to sign.
You will include your completed application form as part of your ONE FINAL PDF DOCUMENT or PRINTED PACKET which you will submit to TRC 3, through a Korean Consulate / Embassy in your home country or through TRC 3’s email address (TBD).

Please NOTE:
You do not need to FULLY complete the Korean application form — only the English form must be FULLY filled out.
However, you must partiallyfill out and hand-sign the Korean form and submit a scanned or printed copy in PDF format as part of your overall TRC 3 submission.

Please note that there is nowhere on the ENGLISH (or KOREAN) TRC 3 application form which specifies where you should include your EMAIL address. We suggest that you include your EMAIL address beneath your physical address in the “ADDRESS” portion of page 1 of the ENGLISH application.

If you submit your application materials by EMAIL to TRC 3 (the email address is TBA), be sure to submit your application materials from the PERSONAL (NOT WORK!) email address you will use THROUGHOUT THE MULTI-YEAR process of TRC 3 (at least 2026-2030)! You can also contact your your nearest Korean Consulate / Embassy regarding where to include your email address on the form.
