This is an example of the formerly secret, KSS internal form titled:
국내소속서류작성의뢰서 / “Request Form for Preparation of Domestic Affiliation Documents”

It is taped to the inside FRONT cover of each KSS Adoptee’s file, and can now be obtained from NCRC through a Petition for Adoption Information Disclosure request (see more information about how to do that below).

Please note that ALL former Korean Adoption Agency files — including those of KSS — are likely to move a SECOND time, sometime in 2026. Please read below for more info.

Even KSS Adoptees who were amongst the 56 who received a TRC 2 (Second Truth and Reconciliation Commission, Republic of Korea) truth finding decision in their cases at the conclusion of TRC 2 in 2025 did NOT obtain (through the TRC 2 process) the formerly secret, internal KSS form called:

국내소속서류작성의뢰서
“Request Form for Preparation of Domestic Affiliation Documents”


This is a form I was the first and only KSS Adoptee to photograph at KSS in my own file folder in 2018.

I am the first and only person to publish anything about this form, for the benefit and understanding of other KSS Adoptees.

I am the ONE person on the planet who is not a KSS employee to have known about this document since 2018. I would gladly have shared information about this and other KSS secrets I know about with TRC 2 had I not been kept out of TRC 2 by supposed “Korean Rights” groups.

Ironically even TRC 2 couldn’t get this document for KSS Adoptees, over 3 years of time.

KSS Adoptees can now obtain this form from NCRC. This is why I am now making information about this form PUBLIC.


The 국내소속서류작성의뢰서 / “Request Form for Preparation of Domestic Affiliation Documents” form is a formerly secret KSS internal document which can now be obtained, along with other KSS documents, from NCRC via a Petition for Information Adoption Disclosure request.

This form was taped into the inside FRONT cover of each KSS Adoptee’s file.

If you are NOT a KSS (Korea Social Service) Adoptee, you will NOT have this document in your Korean Adoption Agency file.

There is a space on the form for birth parent information, though in my case, none was present. Whether or not KSS originally recorded birth parent information on this form will just depend on your case. I have seen another KSS Adoptee’s form which DID contain birth parent information. This information will be partly redacted by NCRC, but it is still well worth obtaining this form — as it was NEVER before possible to do so.

Please note that due to widespread use of Orphanization in Korean Adoption, that even if your ENGLISH Adoptive Child Study Summary says you were “abandoned” and found with a “paper-slip” or “memo” in your “clothings” (sic), or found at a police station, etc. and had “unknown parents”, there is STILL the possibility that this form MAY contain birth parent information. The same is true for KSS’ formerly secret KOREAN Adoptive Child Study Summary.

ALL of these documents can now be obtained from NCRC through a Petition for Adoption Information Disclosure request.

You will get the fastest service if you visit in person in Korea, but you will still need to submit a Petition for Adoption Information Disclosure to NCRC in order to schedule an in-person appointment at NCRC’s Temporary Storage Facility in Goyang, Gyeonggi-do. The only ways to actually get a COPY of your file, however, is to either wait months for your online request to be fulfilled, or to — after first visiting NCRC’s Temporary Storage Facility in Goyang, Gyeonggi-do, to then schedule a SECOND appointment at NCRC’s main office in Seoul.

Please note that sometime in 2026, ALL former Korean Adoption Agency files — including those of KSS — will likely move from NCRC’s Temporary Storage Facility to the Seongam Branch of the National Archives.

This is almost certain to interrupt the plans of some Korean Adoptees who intend to visit NCRC’s Temporary Storage Facility.

The KSS specific form titled, 국내소속서류작성의뢰서 / “Request Form for Preparation of Domestic Affiliation Documents” has NEVER previously been discussed, as KSS Adoptees never previously had access to this document.
